Acute purulent infection localised within the periodontal tissues

Gingival Abscess

Superficial — marginal gingiva only

  • No pre-existing periodontitis
  • Often due to foreign body
  • No attachment loss
  • Localised fluctuant swelling at gingival margin

Periodontal Abscess (true)

Within an existing periodontal pocket

  • Pre-existing periodontitis present
  • Pus drains through pocket or sinus tract
  • Possible acute-on-chronic bone loss
  • Tooth may have increased mobility

Pericoronal Abscess

Around a partially erupted tooth

  • Most common around lower third molar
  • Pericoronal flap involvement
  • Distinct from classic periodontal abscess
