The most common odontogenic cyst, arising at the apex of a non-vital tooth

Classification

Radicular Cyst (Periapical Cyst)

Cyst at the apex of a non-vital tooth, attached to the root.

  • Tooth still present
  • Cyst attached to root apex
  • Non-vital tooth (negative cold/EPT test)
  • Periapical radiolucency on X-ray

Residual Cyst

Persists after extraction of the offending tooth.

  • Tooth has been extracted
  • Edentulous site with radiolucency
  • Cyst not removed at time of extraction
  • May grow undetected for years

Lateral Radicular Cyst

Cyst forms lateral to the root through a lateral root canal.

  • Radiolucency on lateral root surface
  • Tooth present and non-vital
  • Associated with lateral accessory canal
  • Less common variant
